United States

  • Allergy Alert: Windsor Quality Food Co (Toluca, IL) recalls Safeway Select Five Cheese Lasagna (Retail cartons; Package code April 4, 2013) and Safeway Select Meat Lasagna (Shipping cases; Best by date April 4, 2013), because the products contain undeclared egg and soy. The recalled products were manufactured on April 4, 2012 and shipped to retail establishments in Alaska, California, Delaware, the District of Columbia, Hawaii, Idaho, Maryland, Montana, Nevada, Oregon, Texas, Washington, and Virginia.
  • Food Safety Recall: Bay Valley Foods recalls boxed pasta mix products manufactured by its subsidiary, ST Specialty Foods, Inc., because the recalled products include a seasoning blend manufactured by Kerry Ingredients & Flavours that may potentially contain small metal fragments. Affected products were packaged under the Aldi, Coborns, H-E-B, King Soopers, Kroger, Meijer, Mid-Mountain Foods, Mitchell Grocery and Wal-Mart brands.   • Food Safety Recall (FDA Enforcement Report): Corn Products International, Inc. (Westchester, IL) recalls Purimune galactooligosaccharide powder derived from milk lactose (20kg/44.1-lb plastic bags; Product code 113001-156; Lot Nos. 0000117369, 0000117370 and 0000117371; Manufactured by Corn Products Korea, Inc., Icheon-City, Korea), because the product was found to be contaminated with Salmonella. Customers were notified of the recall by telephone on May 10th and May 15th-16th, 2012. The recalled product was distributed to Arizona, California, Nebraska, New Jersey, Pennsylvania, Utah, Vermont, and Washington, as well as to Canada (Ontario and Quebec, only), Czech Republic, New Zealand and the United Kingdom.
  • Dietary Supplement Safety Recall: Botanical Laboratories (Ferndale, WA) recalls Digestive 3 in 1 Health liquid dietary supplement (33.8-oz bottles; Lot 34552C(4-digit time code)A EXP 03/2014) and Digestive 3 in 1 Health liquid dietary supplement (16-oz bottles; Lot 34441C(4-digit time code)A EXP 03/2014), because the products may be contaminated with Salmonella. The recall was initiated after Botanical Laboratories was informed by a raw ingredient supplier (Corn Products International, Inc.) that a sample of one of the ingredients had tested positive for Salmonella.
  • Outbreak Alert: State, local and federal health agencies are investigating an outbreak of E. coli O145 illnesses that has sickened at least 11 people in Georgia, Louisiana, Alabama, Florida and Tennessee. One child died as a result of the her infection. The source of the outbreak is under investigation.

Canada

  • Allergy Alert: Rocky Roc Canada Trade Ltd. (Richmond, BC) recalls Torabika brand Cappuccino Extra Choco Granule (20 x 25g sachets; Product of Indonesia; UPC 8 996001 414019), because the product contains undeclared milk. The recalled product was distributed in British Columbia.
  • Food Safety Recall: Nys Pâtissier, Pâtisserie belge artisanale (Trois-Rivières, QC) recalls Sauce à Spaghetti Maison (1000mL glass jars), because the product was not manufactured in a manner that would ensure its microbiological safety.

  • Food Safety Recall (Germany): Fa. Heiderbeck Käse vom Feinsten GmbH (distributor) and Casarrigoni srl (manufacturer) recall La Dolce Crema – “Spoon Gorgonzola” cheese (Product of Italy; Expiration dates 07.06./15.06./21.06./28.06), because the product may be contaminated with Listeria monocytogenes.
  • Food Safety Recall (Greece): EFET orders the recall of Kouniniotis Iarmony Black Currant (200g packages; Expiry date 12/2012; Batch L 1356DSPG), due to contamination with unsafe levels of Ochratoxin A.
  • Food Safety Recall (Greece): EFET orders the recall of TRS – Tumeric Powder (100g; Expiry 31.03.2014; Product of India; Packed by TRS Wholesale Co. Ltd, UK), due to Salmonella contaimation. The recalled Tumeric Powder was sold by Sri Lanka Asien Market, Athens.

  • Food Safety Recall (Australia): Jmark Pty Ltd., trading as Gourmet Selections, recalls Gourmet Selections Greek Salad (150g and 300g plastic tubs; use by dates from 6 JUN 12 to 14 JUN 12 inclusive), due to E. coli contamination. The recalled product was sold from Woolworths, IGA and other independent supermarkets in Western Australia.

  • Medical Device Safety Recall (Australia): The Therapeutic Goods Administration (TGA) advises that Other-Sonic Generic Ultrasound Transmission Gel (Product of USA; 250ml and 5L containers; Lot Nos. 060111, 090111 and 081411) has been recalled due to bacterial contamination. The affected lots were manufactured by Pharmaceutical Innovations Inc., USA between June and December 2011, and were the subject of a recall by the manufacturer in March 2012. TGA has confirmed the presence of the bacteria Klebsiella oxytoca and Pseudomonas aeruginosa in samples from the contaminated lots.

  • Food Safety Recall: Giant Food Stores advises its customers that The Campbell Soup Company has recalled Campbells Oyster Stew Soup, Condensed (10.5-oz cans; UPC 5100003087), based on FDA’s findings of insanitary conditions of shellfish areas in Korea.
  • Food Safety Recall: Bi-Lo advises its customers that the manufacturer has recalled GrandAsia Oysters (8-oz can; UPC 19882-00204 or 19882-0204; All lots).
  • Prescription Pharmaceutical Product Recall: Sandoz recalls Introvale® Oral Contraceptive (Lot nos. LF00478C, LF00479C, LF00551C, LF00552C, LF00687C, LF00688C, LF00763C, LF00764C, LF00765C and LF01261C), due to a packaging flaw. The recalled lots were distributed only in the USA between January 2011 and May 2012.

Canada

  • Food Safety Recall: Kabul Farms (Mississauga, ON) recalls Ground Beef Reg. and Ground Beef Lean (varying weight plastic bags; sold on May 31, 2012), because the products may be contaminated with E. coli O157:H7. No confirmed illnesses are associated with these products at this time. The recalled products were sold only from the Kabul Farms store at 255 Dundas Street West, Mississauga, ON.

Europe

  • Food Safety Recall (Sweden): Statoil recalls Smoked Salmon and Asparagus Salad (Sold May 21st through June 5th), after Listeria was reported in the smoked salmon that was used in the salad. The recalled salads were sold at Statoil Stations.
  • Food Safety Recall (Denmark): Skare East Ltd. recalls Minced Pork and Veal, 3-7% (500g pkgs; Package date 30.05.2012; Best before 07.06.2012), after Salmonella was detected in samples of the product.
  • Food Safety Recall (Denmark): Danish Supermarket A/S recalls Organic Cane Sugar (Packed in Germany; Production date 10/4-2012; Best before 10/4-2015), due to contamination with mouse droppings, cardboard pieces and plastic strips.
  • Food Safety Recall (Greece): New Provis SA recalls Provis Loutraki Natural Mineral Water (0.5L; Expiration dates 10/1/13, 13/01/13, 15/5/13 and 18/5/13), due to contamination with off-white suspended particles of filamentous hyphae (mold).

  • Allergy Alert: BaLe Meat Processing Wholesale, Inc. (Chicago, IL) recalls BaLe Cha Bi (Skin Pork Patty Roll), BaLe Cha Lua (Steamed Pork Patty Roll), BaLe Cha Chien (Fried Pork Patty Roll), BaLe Cha Hue (Spicy Pork Patty Roll), BaLe Cha Lua La Chuoi (Steamed Pork Roll Wrapped in Banana Leaves), and BaLe Cha Lua (Raw Pork Roll finished product), because of misbranding and the presence of undeclared wheat. The recalled products, bearing establishment number EST. 18442, were shipped to retail locations in Oakland, California; Denver, Colorado; Chicago, Illinois; Dorchester, Massachusetts; and Minneapolis, Minnesota.
  • Food Safety Recall: Blackstar Dairy (Solway, MN) recalls Blackstar Dairy brand Heavy Whipping Cream (Sell-by date 6/17/2012), after routine sampling by the Minnesota Department of Agriculture found the product had not been properly pasteurized. The recalled product is distributed statewide in Minnesota by Trudeau Distribution.
  • Food Safety Recall: Crown Prince recalls Crown Prince Whole Boiled Oysters (UPC 0 73230 00052 2), Crown Prince Naturral Whole Boiled Oysters (UPC 0 73230 00852 8), Crown Prince Natural Smoked Oysters in Olive Oil (UPC 0 73230 00851 1) and Crown Prince Natural Smoked Oysters with Chili Pepper (UPC 0 73230 00866 5), because FDA has declared that all shellfish from South Korea are considered to be adulterated. This recall only affects oysters from South Korea. No other Crown Prince items or countries of origin are affected.
  • Food Safety Recall: Lowes Foods recalls Reese Petite Smoked Oysters (3.7 oz can; Product of South Korea; UPC 70670 00575), because FDA has removed Korea from its list of certified shippers, due to potential contamination.
  • Dietary Supplement Safety Alert: FDA warns consumers that Reumofan Plus, which is manufactured in Mexico by Riger Naturals, contains several active pharmaceutical ingredients, which are not listed on the label and which could be harmful. FDA has received multiple reports of averse events associated with the use of this product.

Canada

  • Food Safety Recall: Boucherie Claude Giguère (Trois-Rivières, QC) recalls prepared meals packed in glass Mason jars (500mL jars), because the products were not stored under refrigeration at all times and may be unsafe for human consumption. The products are missing the label instruction “Garder réfrigéré”/”Keep refrigerated.”
  • Food Safety Recall: Les Aliments 2000 Inc. ((Saint-Augustin-de-Desmaures, QC) recalls Cuisine du Viêt-nam brand Butterfly Knots Honey & Sesame (102g packages; UPC 8 59912 00102 1; all lots), because the product may be contaminated with Salmonella. The recalled product was distributed in Quebec.
  • Food Safety Recall: Pusateri’s Fine Foods (Toronto, ON) recalls Delicately Smoked Boneless Rainbow Trout (Packaged on 2012.05.07/Best before 2012.05.21 and Packaged on 2012.05.13/Best before 0212.05.27), Nanuk Balick Salmon Centre Cut Atlantic Smoked Salmon (Packaged on 2012.04.19/Best before 2012.05.19; Packaged on 2012.05.03/Best before 2012.06.02; Packaged on 2012.05.11/Best before 2012.06.12) and Original Lox Smoked Sockeye Salmon (200g; Packaged on 2012.05.11/Best before 2012.06.11 and Packaged on 2012.05.15/Best before 2012.06.05), because the products may be contaminated with Clostridium botulinum. The recalled products were sold only at Pusateri’s Fine Foods (1539 Avenue Road, Toronto, ON).
  • Food Safety Recall: M & M Meat Shops Ltd. recalls M & M Meat Shops Chicken Phyllo Collection (288g; Lots 3411B, 3421A and 3421B; UPC 6 61815 00003 8), due to contamination with glass particles. The recalled product was sold in Manitoba, New Brunswick, Newfoundland and Labrador, Nova Scotia, Ontario, Prince Edward Island, and Quebec.
  • Food Safety Recall: Costco Wholesale recalls Kirkland Signature Lemon Cupcakes, Kirkland Signature Red Velvet Cupcakes and Kirkland Signature Mix and Match Muffins (produced in the St. Catherines’ Costco warehouse between May 22nd and May 24th, 2012), because the product may contain a bolt or washer.

Europe

  • Food Safety Recall (France): La Fromagerie des Sources (Lozère) recalls Berger des Sources brand Tommettes de Brebis, Meules de Brebis, Tomes de Brebis, Bleu de Brebis and Fourme de Brebis cheeses (All lot numbers; Expiry dates May 22, 2012 to March 12, 2013, inclusive) due to Salmonella contamination. The recalled cheeses were sold, both labelled and unlabelled, in stores and markets in the following départements: 7, 17, 16, 22, 24, 26, 30, 31, 33, 34, 37, 38, 40, 42, 43, 48, 58, 59, 69, 75, 83, 84, 85, and 87.
  • Food Safety Recall (Belgium): SPRL Fromagerie du Pont de Pierre recalls Louché de chèvre goat cheese (Lot 143; Best before 27/06/2012; sold from 24/05/2012 to 31/05/2012), because the product contains Listeria monocytogenes. The recalled product may also have been distributed to France and Luxembourg.

Latin America and the Caribbean

  • Dietary Supplement Safety Alert/Recall (Mexico): The Secretary of Health has ordered the recall of Reumofan Plus (Manufactured by Riger Natural, Jalisco) from the market, because this product, which is claimed to be “100% natural” has been found to contain the pharmaceutical ingredient Dexamethasone (dexametasona), a potent antiinflammatory and immunosuppressor, in addition to methylprednisolone (metilprednisolona). Neither of these pharmaceutical ingredients are declared on the product label, and the product itself is not a registered health product. The recalled product was sold in Jalisco and Baja California, and may also have been sold elsewhere, including the USA.
