Recalls and Alerts: October 6, 2011

United States

  • Cosmetic Product Safety Alert: The Arkansas Department of Health warns consumers that a number of mercury poisoning cases in South Texas appear to be linked to the use of Crema Aguamary, a cosmetic produced in Mexico and not approved for use in the USA. Use of this product can result in elevated blood levels of mercury.

Canada

  • Allergy Alert Amended: The September 30th Allergy Alert issued by CFIA has been amended to correctly identify the undeclared allergens present in the recalled Kimchi products, and to provide additional distribution information. The following products of Korea were recalled by the Canadian importers, due to the presence of undeclared fish, shrimp and/or sesame: Chongga brand Kimchi Richi (80g), Daesang FNF Corporation Gat Kimchi (500g), and Daesang FNF Corporation Godulbbaegi (200g). The products are known to have been distributed in Alberta, British Columbia, Ontario, Manitoba, New Brunswick and Nova Scotia, but may also have been distributed nationally.
  • Food Safety Recall: Foodfest International 2000 Inc. (Concord, ON) recalls Strubs ready-to-eat Danish Style Smoked Grav-Lox Atlantic Salmon (7 oz/200g packages; Lot code 09262011), because the product may be contaminated with Salmonella. The recalled Grav-Lox was distributed in Ontario.
  • Food Safety Recall: Brittles ‘N More Ltd. recalls Brittles ‘N More Peanut Brittle (200g; Code 729081), due to the presence of glass particles. The recalled product was sold in British Columbia, Ontario and Quebec.
  • Food Safety Recall: Okanagan Spring Brewery Ltd. recalls Okanagan Spring Brewmaster’s Black Lager (6 x 341 mL; codes J0911 (1700 to 1815) V), due to the presence of process residues. The recalled product was sold in British Columbia.
  • Outbreak Alert: CBC News reports that six patients and one employee at the Kenneth E. Spencer Memorial Home in Moncton, NB were infected with Salmonella in an outbreak that started September 18th. One resident of the Home has died.

Europe

  • Allergy Alert (UK): Tesco recalls Tesco Coffee and Walnut Cake, 380g (Use by 07/10/11, 09/10/11, and 10/10/11) due to the accidental inclusion of pecans, which were not listed in the recipe and are not declared on the product label.
  • Food Safety Recall (Netherlands): On August 10, 2011, Hickory Smokehouse Miami, Inc. (Hialeah, FL, USA) recalled 25 cases of Fjord Superior Norwegian cure Smoked Salmon / Hickory House Superior Smoked Salmon Sliced retail pack (Batch 24182), after the Florida Department of Agriculture found Listeria monocytogenes in a sample of the product. The recalled Smoked Salmon was exported to the Netherlands. FDA issued a Warning Letter to Hickory Smokehouse on August 1st, detailing a long list of “significant deviations to the seafood HACCP and food CGMP regulations” and misbranding issues.
  • Food Safety Recall (Denmark): Cervera A/S recalls Worchester Sauce (1L glass bottle; Best before 10th June and 16 June 2013), because fermentation has occurred in some bottles.
  • Food Safety Notification (EU #2011.1365): Diarrhoeic shellfish poisoning toxins in chilled mussels from Italy; distributed to Germany, Italy and Switzerland.
  • Food Safety Notification (EU #2011.1367): Ochraoxin A in wede aba from Ghana; distributed to the United Kingdom.
  • Food Safety Notification (EU #2011.1368): Salmonella enteritidis in frozen chicken fillets from Brazil, via the Netherlands; distributed to Denmark.
  • Food Safety Notification (EU #2011.1369): Insect infestation in roasted and salted pistachio nut kernels from the United States, via Slovakia; distributed to the Czech Republic.
  • Food Safety Notification (EU #2011.1370): Bacillus cereus in couscous from the United Kingdom; distributed to Austria.
  • Food Safety Notification (EU #2011.1371): Oxamyl in cucumbers from the Czech Republic; distributed to Slovakia.
  • Food Safety Notification (EU #2011.1372): Chlormequat in pears from the Netherlands; distributed to Slovakia.
  • Allergy Alert Notification (EU #2011.1373): Undeclared soya in corn flakes from Germany, via the Czech Republic; distributed to the Czech Republic and Slovakia.

Repeat Offenders – Recurring Recalls

Thumb Oilseed Producers’ Cooperative (Ubly, MI) recalled soybean flour and soy meal today, due to possible Salmonella contamination. The recalled products were distributed to wholesale customers in Illinois, Vermont, Minnesota, Pennsylvania, Wisconsin, New Hampshire and Canada, for use in the manufacture of human and animal food.

This is the second time in less than two years that Thumb Oilseed has run afoul of Salmonella.

On February 25, 2010, Thumb Oilseed Producers’ Cooperative (Ubly, MI) recalled shipments of soybean grits and flour, because the soy products had “…the potential to be contaminated with Salmonella…” The recalled soybean grits and flour were distributed to a “limited group” of wholesale customers in the USA.

According to Thumb’s recall notice, the recall resulted from “…routine sampling programs conducted by the company and its wholesale customers.” The company did not reveal that FDA had been inspecting Thumb’s manufacturing facility since January 28th, and had completed its inspection on February 23rd – two days prior to the date of the recall notice.

In their Form 483 (Inspectional Observations) report, handed to the company at the completion of the inspection, FDA investigators documented a serious of failures on the part of Thumb, including an egregious disregard for public safety, as witnesses by the details of Observation 1, as follows (emphasis added):

Food which has become contaminated to the extent of being adulterated within the meaning of the Act is not rejected or if permissible, treated or processed to eliminate the contamination..

Specifically, in the last fourteen months of production of lots of Non-GMO soy flour, your firm released finished product non-GMO soy flour found to be positive for Salmonella sp. on two separate occasions.

a. On 12/22/08, you had a private laboratory analyze Non-GMO soy flour lot TF121608 for Salmonella sp. The lab reported this lot was positive for Salmonella sp. on 12/22/08. The lot was subsequently retested on 01/12/09 by the same private laboratory, and released into commerce in four different deliveries: {quantity redacted} 40-1b. bags on 01/13/09; {quantity redacted} 40-1b. bags on o2/04/09; approximately {quantity redacted} 40-lb.bags on 09108/09; and {quantity redacted} 40-1b. bags on 03/27/09.

b. On 10102/09, you had a private laboratory analyze Non-GMO soy flour lot TF092409 for Salmonella sp. The lab reported this lot was positive for Salmonella sp. 10/02/09. The entire lot, a total of 4,random weight totes, was subsequently released into commerce as animal feed on 10/19/09.

Other observations reported by the FDA inspection team included:

Observation 2: Failure to manufacture, package, and store foods under conditions and controls necessary to minimize contamination.

Observation 3: Failure to conduct cleaning and sanitizing operations for utensils and equipment in a manner that protects against contamination of food-contact surfaces.

Observation 4: Failure to maintain buildings, fixtures, or other physical facilities in a sanitary condition.

Observation 5: Effective measures are not being taken to exclude pests from the processing areas.

The firm’s response to FDA’s observations evidently was not pleasing to the agency; FDA issued a formal Warning Letter to Joann F. Rutkowski, COO of Thumb Oilseed on June 24, 2010. In its letter, FDA informs Ms. Rutkowski that its lab analyses found Salmonella in forty-three locations inside Thumb’s manufacturing plant. And FDA recovered Salmonella from a sample of the company’s finished product soy flour.

All of the Salmonella found in the plant’s environment and in the finished soy flour were a single serotype – Salmonella senftenberg – and all produced indistinguishable “DNA fingerprint” patterns.

The Warning Letter ended by requesting that the company “…notify this office in writing, within 15 working days from your receipt of this letter, of the current status of your corrective actions, including any further specific steps that you have taken to correct the noted violations. If you cannot complete all corrections before you respond, we expect that you will explain the reason for your delay and state when you will correct any remaining violations.”

Whatever steps Thumb Oilseed may have taken in the past year to correct all of their breaches of sanitation and Current Good Manufacturing Practices clearly weren’t enough. And its failure to clean up a dirty mess will be expensive for itself, its customers, and consumers – who ultimately pay the bill.

United States

  • Allergy Alert: Flowers Foods recalls Mrs. Freshley’s Canadian Label Apple Cinnamon Fruit and Pastry Cereal Bars, Mrs. Freshley’s Canadian Label Blueberry Fruit and Pastry Cereal Bars, and Mrs. Freshley’s Canadian Label Strawberry Fruit and Pastry Cereal Bars, because the cereal bars may contain undeclared milk. The recalled bars were sold in discount stores nationwide in dual-language (French/English) multipack cartons.
  • Food Safety Recall: Thumb Oilseed Producer’s Cooperative (Ubly, MI) recalls the following soybean flour and soy meal (used to manufacture human and animal food), due to Salmonella contamination: Nex Soy (40-lb bags; Lot Nos. TF112310 through TF033011), Soy Beginnings (40-lb bags; Product Code 285100-NFB; Lot numbers TF112310 thru TF033011), Soy Beginnings (1500-lb polyurethane totes; Product Code 285100-NFT, Lot numbers TF112310 thru TF082311), and soy meal (0.08-ton loads after custom processing; Lot Nos. O011711 through O081711). The recalled soy products were distributed to a “limited group” of wholesale customers in Illinois, Vermont, Minnesota, Pennsylvania, Wisconsin, New Hampshire and Canada in November 2010 through September 2011. The recall is the result of routine sampling conducted by the company and FDA, which found Salmonella both in finished product and in the manufacturing environment.
  • Food Safety Recall Update: USDA releases retail distribution list for beef products recalled by Manning Beef, LLC due to E. coli O157:H7 contamination.
  • Dietary Supplement Safety Recall: Superior Metabolic Technologies, Inc. (Marietta, GA) recalls Uprizing 2.0 testosterone booster (black plastic bottle containing 90 capsules; all lots), because tests have shown the product to contain superdrol, a synthetic steroid banned by FDA. Uprizing 2.0 was sold through retail health food stores in Alabama, Arizona, California, Georgia, Mississippi, New Jersey, South Carolina, and Texas.
  • Food Safety Alert: Missouri Department of Health and Senior Services (DHSS) has embargoed retail meat products sold by Reis Meat Processing (Jackson, MO), after routine inspection revealed sanitation concerns. DHSS has begun testing on retail meat products processed and packaged by Reis.

Canada

  • Allergy Alert: Flowers Foods (Thomasville, GA) recalls the following Mrs. Freshley’s brand multipack Cereal Bars labelled for the Canadian market, because the products may contain undeclared milk: Apple Cinnamon Fruit & Pastry Cereal Bars (170g (6 x 28g); Blueberry Fruit & Pastry Cereal Bars (170g; 6 x 28g) and Strawberry Fruit & Pastry Cereal Bars (170g (6 x 28g). The recalled cereal bars were distributed in Ontario and Quebec, and may have been distributed nationally.
  • Food Safety Recall: Gagan Foods International Ltd. (Delta, BC) recalls Gagan Coriander Powder (100g, 200g, 400g & 1 kg sizes; Product of India; Batch No. RSLI/09-10/DG-13), because the product may be contaminated with Salmonella. The recalled coriander powder was distributed in Alberta, British Columbia and Manitoba.

Europe

  • Allergy Alert (Ireland): Lindt and Sprüngli (UK) Ltd. withdraws Lindt Lindor White Chocolate Truffles (200g; batch numbers: L4711, L2681, L2621 and L3691; best before dates: 31/07/2012 and 31/08/2012), because the product may contain traces of  undeclared hazelnuts and almonds.
  • Food Safety Recall (Denmark): Ardo A/S recalls Frozen Whole Raspberries (375g; Package date 25.12.2010; Best before 25.12.2012), because the berries may be contaminated with Norovirus.
  • Food Safety Recall (Denmark): Danfrugt Skaelskor A/S recalls Pure Bites Apples and Pure Bites Pears (100g bags; Lot #265; Best before 7 October 2011), because the fruit may be contaminated with Listeria monocytogenes.
  • Food Safety Notification (EU #2011.1359): Salmonella spp. in salami from Germany; distributed to Germany.
  • Allergy Alert Notification (EU #2011.1360): Traces of wheat in oat flakes used in oats and muesli from the Netherlands, with raw material from Sweden, via the United Kingdom; distributed to the Netherlands and the Untied Kingdom.
  • Food Safety Notification (EU #2011.1361): Foodborne outbreak suspected (scombroid syndrome) to be caused by chilled tuna fillets from Spain; distributed to Italy.
  • Allergy Alert Notification (EU #2011.1362): Undeclared hazelnut and almond in white chocolate truffles from Italy; distributed to Ireland and the United Kingdom.
  • Food Safety Notification (EU #2011.1364): Salmonella in chilled smoked salmon from France; distributed to Italy.
