“Your Quality Unit has failed in the responsibility and authority to monitor Quality Systems designed to assure the quality of drug products manufactured and packaged at your firm.”
– FDA to Novartis Consumer Health (Lincoln, Nebraska), July 8, 2011
On July 8, 2011, FDA completed an inspection of Novartis Consumer Health (Lincoln, Nebraska), and handed the company a report (Form 483) listing 13 separate “Observations” relating to deficiencies in Novartis’ quality assurance and complaint handling systems.
About one month later, Giant® Food Stores notified its customers that Novartis had recalled Excedrin Extra Strength Caplets, 100-count (Lot #10068948; Expiration date March 31, 2010), “as a result of instances of product mix-ups found in the lot” (posted on eFoodAlert September 7, 2011).
On November 1, 2011, Giant® Food Stores warned its customers of another Novartis recall – Excedrin – Pain Reliever Acetaminophen Adult, 100 CO (Lot #10087530; Expiration date 3/31/2013). Again, the recall was due to product mix-up occurrences in the lot. That same day, Food City posted a recall notice for three Novartis products, including Excedrin Extra Strength Caplets 24 count, Excedrin Migraine Caplets 24 count, and Excedrin Floorstand ES Migraine with $1 IRC. More retailers chimed in the following day, and a consolidated list of recalled lots was provided on eFoodAlert’s Recalls and Alerts: November 2, 2011.
Today (January 8, 2012), in an unusual Sunday announcement, Novartis Consumer Health, Inc. issued a News Release detailing the recall of “…all lots of select bottle packaging configurations of Excedrin® and NoDoz® products with expiry dates of December 20, 2014 or earlier as well as Bufferin® and Gas-X Prevention® products with expiry dates of December 20, 2013 or earlier, in the United States. NCH is taking this action as a precautionary measure because the products may contain stray tablets, capsules, or caplets from other Novartis products, or contain broken or chipped tablets.”
Am I being silly, or is it just possible that FDA’s posting on January 5th of the July 8, 2011 Novartis Inspectional “Observations” Report might be somehow tied into this sudden decision by Novartis to go public?
Here are just a few excerpts from the report. These are direct quotations. Read them and decide for yourself.
- You have failed to open deviation investigations into numerous “critical” consumer complaints of foreign products found inside the drug products manufactured at your firm.
- You have failed to adequately investigate 166 complaint instances of foreign tablets in your drug products since 2009.
- You have failed to extend investigations to all batches of product potentially affected by a problem.
- You have received numerous consumer complaints which were not submitted to the FDA as required by your firm’s procedures.
- Your Quality Assurance review of critical foreign tablet complaint investigations (Technical Complaint Investigation Reports) is not occurring in a timely manner… There are approximately 138 reports that took over 100 days to review.
- You have failed to identify the root cause of customer complaints for solid dosage form foreign tablets of products manufactured at your firm since at least 2009.
- Your Unplanned Deviation Reports are deficient in that conclusions drawn are not supported by the evidence in the reports.
- Your Quality Assurance Unit has consistently failed to review critical complaints for drug products manufactured and packaged at your facility. For example [for 2011], 223 critical complaints have not been properly reviewed out of 223 critical complaints received by your firm… Also, your Quality Assurance review of critical complaints ceased in mid-October 2010. This deficiency was unnoticed by your Quality Unit until this FDA inspection.
The entire report is 13 pages long. It details lapses in procedure, in judgment, and in training.
In addition to the recall, Novartis advises that its Lincoln, NE facility “…has voluntarily suspended operations and shipments to accelerate maintenance and other improvement activities at the site.”
I have to ask – WHAT TOOK SO LONG?

I called Novartis Consumer Health, Inc. in April, 2012 to see when they thought all excedrin products would be back on the shelves & I was told sometime in July 2012, the recall would be over & the products put back on the shelves. They even called me back & asked me questions as to why I took Excedrin Extra Strength & how many I took per month. It was a big wooptedo as to the reasons I called & they even asked personal health questions, & wanted to know my Drs name & ph. #. Odd Huh??? They sent papers for me to fill out too. I just threw them away. I do hope July is correct though. Nothing works on me except Excedrin. Not even Bufferin.

I wish they would hurry n fix the problem so they could return excedrin migrain back to retailers i suffer from really severe migrains my doctor prescribed me imitrex they didnt work i have tried several other medications with no success i ask that someone plz if they no im open to suggestions cause in my experience excedrin is the only thing that relive my migrains.

Considering the comments of Al Armendariz of the EPA exposed last week about Crucifying Victims and looking at the charges levied against the Lincloln, NE Novartis facility, I wonder if the FDA was taking lessons from the EPA. I have used Excedrin for many years with not a hint of a problem. I am wondering who regulates the regulators? Congress?

I received the recall notice from Costco stating 4 different item numbers were purchased by me since 2009 (of Excedrin). Of course, I no longer have the bottles to get UPC’s from. I have experienced rashes on my skin upon taking it and now am relating the two.
How do I go about getting refunds and submitting my symptoms if I no longer have the bottles? Who keeps empty aspirin bottles from 2009?

My husband and I have taken Excedrin off and on for years. On Dec.
6th, 2011, I went to the emergency room for a sprained knee. I took
Excedrin for the pain. On January 2, 2012, I was rushed to the ER
because my bladder had stopped functioning. It is January 13th and
I am still on a catheter, at home and unable to work. I am highly
allergic to hydrocodone. Could the Excedrin I took have been mixed
with hydrocodone? My Dr.’s are baffled regarding my bladder problem
as I have never had a bladder problem in my life. I am otherwise a
healthy 44 yr. old.

Leslie, it is possible. Zydone – one of the Endo prescription products included in the mix-up warning – is a hydrocodone product (see http://www.fda.gov/Drugs/DrugSafety/ucm286226.htm). There also are four oxycodone-based products (chemically related to hydrocodone) in the warning list. Did you notice whether any of the pills in the Excedrin bottle were of a different size, shape or color? I strongly urge you to speak with your Dr. and contact the company AND FDA for more information.

I have used Excedrin Migrane, geltabs and tablets for a long time, I have some currently that have been purchased over the last 3-4 months.
Need I be concerned? I have not noticed any flaws or different pills in
any… also I can not find on your info, what procedure we are to do
for recall, I do not have the original boxes, of course, but lot numbers
are on the bottles.

I had purchased my 300CT from Sam’s Club about 6 months ago- They just called and left a message today. Can someone tell me what the complaints are about??? Why are they recalling this??? We have took anout one full bottle between me and the wife.

Robert, the recall is due to serious Quality System issues at the Novartis (Excedrin, etc) manufacturing facility in Lincoln, Nebraska. The company received numerous reports of pill mix-ups (a few pills of one type getting mixed into a bottle that contains a different type of pill. They failed (according to the FDA inspection report) to act on these mix-ups and fix the source of the problem. This recall is because they have no idea how extensive the pill mix-up may be. I have one of these products in my home right now, but I don’t have the packaging so I don’t have the UPC code. The lot number and expiration date is on it, but how do I know if it is in this recall?

Here is a link to a pdf document that includes several numbers related to the products being recalled. I only had the bottle and the only number that was available to me was the NDC code that was on the label. So first look for the exact product name, and then check for the NDC code. Good luck.
